Travel Vaccinations and advice

Our Practice Nurses run Travel Clinics, please click this link to access a Travel Questionnaire and either email this back to holsho.practice@nhs.net OR send as an attachment using the Contact Us Online with an admin query. 
If you are unable to complete this or print and complete see reception for a copy and return to reception.

Our Nursing team will contact you directly to arrange a convenient appointment.

Please note this MUST be received at least 6 weeks prior to travel.

Some countries require proof of vaccination (for example, for polio or yellow fever vaccination), which must be documented on an International Certificate of Vaccination or Prophylaxis (ICVP) before you enter or when you leave a country.
